How to Authorize a Prescription
Information to be included:
  • Client Name
  • Client Address (with zip code)
  • Client Phone
  • Date
  • Pet Name
  • Pet Species
  • Medication name
  • Strength
  • Directions for use
  • Doses authorized (number of tabs, capsules, etc.)
  • Number of refills authorized
  • Authorization for generic equivalent (if available)
As the prescribing veterinarian, we will also need your name and telephone number, and would appreciate your fax number.

Submitting Prescriptions
You may submit prescriptions in any of the following ways:
  • Fax the prescription to our pharmacy using our downloadable Prescription Fax Form.
    Our fax number is: 1-800-447-2404
    Faxes are only accepted from veterinarians, and only when a valid veterinarian-client-patient relationship exists.
    (You will need Adobe Acrobat Reader to view/download our Fax form. Adobe Acrobat is free.)
  • Call our pharmacy with the prescription - 1-800-447-3021
    If calling after hours, you are welcome to leave a confidential voice message for our pharmacists.
  • Mail the prescription to us at the address above.
    Or you may provide your client with a written prescription containing the information above and have your client mail it to us.
Pharmacy Services Offered to Veterinarians
  • All prescriptions will be individually labeled with the dosage directions you provide us.

  • Prescriptions will include a Patient Information Sheet containing an overview of the prescribed medication, and detailing pertinent information such as storage recommendations, possible side effects, common drug and food interactions, and more.

Accreditations and Licensures

Drs. Foster and Smith Pet Pharmacy is licensed in Wisconsin (WI License No. 9291-42) and in all states requiring out-of-state licensure. Veterinarians have an opportunity to consider receiving an Authorization Fee for each prescription food order filled by Drs. Foster and Smith to the extent consistent with all applicable professional standards.

Before enrolling in the program, each veterinarian is encouraged to consult their applicable state professional guidelines as to whether their acceptance of this fee is permissible.
