How to Authorize a Prescription
Information to be included:
- Client Name
- Client Address (with zip code)
- Client Phone
- Date
- Pet Name
- Pet Type (dog, cat, horse, etc.)

- Medication name
- Strength
- Directions for use
- Doses authorized (number of tabs, capsules, etc.)
- Number of refills authorized
- Authorization for generic equivalent (if available)
- Authorization for custom compound option

As the prescribing veterinarian, we will also need your name and telephone number, and would appreciate your fax number.
Submitting Prescriptions
You may submit prescriptions in any of the following ways:
- Fax the prescription to our pharmacist using our downloadable Prescription Fax Form.

Faxes are only accepted from veterinarians, and only when a valid veterinarian-client-patient relationship exists.

If calling after hours, you are welcome to leave a confidential voice message for our pharmacist.
- Mail the prescription to us at the address above.
Or you may provide your client with a written prescription containing the information above and have your client send it to us.
We will send the prescription to the client unless you direct us to send the prescription to you.
Services Offered to Veterinarians
- All prescriptions will be individually labeled with the dosage directions you provide us.
- Prescriptions will include a
Patient Information Sheet containing an overview of the prescribed medication, and detailing pertinent information such as storage recommendations, possible side effects, common drug and food interactions, and more.
- Our full-service
compounding pharmacy can formulate pet medications in the exact strength, size, dosage form and flavor you prescribe - even hard-to-find or discontinued medications.
