How to Authorize a Prescription
Information to be included:
- Client Name
- Client Address (with zip code)
- Client Phone
- Date
- Pet Name
- Pet Species
- Medication name

- Doses authorized (number of tabs, capsules, etc.)
- Number of refills authorized
- Authorization for generic equivalent (if available)

Submitting Prescriptions
You may submit prescriptions in any of the following ways:
- Fax the prescription to our pharmacy using our downloadable Prescription Fax Form.

Faxes are only accepted from veterinarians, and only when a valid veterinarian-client-patient relationship exists.

If calling after hours, you are welcome to leave a confidential voice message for our pharmacists.
- Mail the prescription to us at the address above.
Or you may provide your client with a written prescription containing the information above and have your client mail it to us.
Services Offered to Veterinarians
- All prescriptions will be individually labeled with the dosage directions you provide us.
- Prescriptions will include a
Patient Information Sheet containing an overview of the prescribed medication, and detailing pertinent information such as storage recommendations, possible side effects, common drug and food interactions, and more.
